Why a questionnaire?

  • A questionnaire is used as a primary support in gathering data.
  • A questionnaire offers the respondents anonymity.
  • A questionnaire offers the most familiar, convenient and least intrusive form of direct response.
  • A questionnaire is the most cost-effective means of market research.
  • A questionnaire reduces bias in its ability to present uniform questioning.
  • A questionnaire is usually brief so respondents spend no more than 10-15 minutes completing them.

How to fill out this survey

Experience levels are as follows:
NONE
LITTLE experience (but not enough, in your judgement).
SUFFICIENT experience for performing your duties.
EXTENSIVE experience that goes beyond your present duties.

  • Your answers will assist in the continuing effort to provide better training.
  • Your answers will be handled in the strictest of confidence.
  • Your answers will be tabulated with those of others to determine training needs, content requirements and format preferences.
  • The questionnaire will present a multi-layered offering of skills with which you may or may not be acquainted, so please be candid and thorough in completing this questionnaire.
  • Data collected will be used to design future training classes that you and your fellow employees will receive.
